Mission Statement
The mission of the athletic training staff at Mary Baldwin University is to provide the best quality medical care for its student-athletes with the ultimate goal being the safest and fastest return to play as possible. This goal has been and will continue to be achieved through the proper prevention, education, care, and rehabilitation of athletic injuries by a dedicated and knowledgeable sports medicine team, including the area physicians, head athletic trainer, and coaches.
Are You Ready To Play?
All incoming and returning athletes must have an athletic physical and submit the Athletic Medical Eligibility Forms and Pre-participation Physical Exam to the athletic trainer. This is in addition to the student health form that must be turned in to the health center. Both should be completed at the same time. If you have any questions, please contact the Head Athletic Trainer.
Mandatory Forms:
1. Pre-participation (first-year or transfer students) or Returning Student Questionnaire (returning students only)
2. Sickle Cell (year 1)
4. Medication Exemption Information
7. Rules of the Training Room (read only)
When you have finished filling out the Athletic Medical Eligibility Forms and completed all exams, please bring items 1,2,3 to your first day meeting and deliver to Marah Jones, Head Athletic Trainer. Should you have any questions, please contact her at the following email.
